Leaving hospital is often the week a household needs an extra pair of hands. The hospital discharge is a medical decision. What Konex Care can add, once you are home, is practical support: showering, dressing, meals, and help around the house.

What a home visit can cover

The detail is on our post-hospital care page. In short, it is personal care, meals and help to keep the house safe while you recover. Clinical nursing tasks, where they are needed, are described separately under nursing assistance. We do not replace the hospital team.

When to call

In most cases support can be ready for the day you leave hospital, if we are contacted before discharge. Ask your discharge planner to refer you, or call us on (03) 8524 7699. Support lasts as long as you need it, from a few days to several weeks, and we adjust it as recovery progresses.

Hospitals families ask about

Three public hospitals come up because they are close to our Murrumbeena office. Caulfield Hospital is in Caulfield. Monash Medical Centre is in Clayton, east of Oakleigh. Moorabbin Hospital is in Bentleigh East. Konex Care does not work on those wards and does not book visits inside the hospital. If the discharge address is one we can reach, we visit the house.

Physiotherapy after a fall or an operation

Physiotherapy at home and occupational therapy at home are separate visits. You do not need a GP referral to book privately or through aged care or NDIS funding. A GP referral is needed if you want to claim Medicare rebates under a Chronic Condition Management Plan.
