Caulfield is north of Murrumbeena. Caulfield station, Caulfield Park and the racecourse precinct are the landmarks, and Caulfield Hospital is in the suburb.
We do not work on the hospital wards, and we are not an outreach team for Caulfield Hospital. Once someone is home at a Caulfield address we can cover, the support is practical: showering, dressing, meals, and someone in the house while the family is at work. Physiotherapy and other allied health can be booked as a separate home visit.
Caulfield, Caulfield North and Caulfield South are not the same trip from Poath Road. Call with the street and we will tell you if we can take it.

Questions about care in Caulfield
Do you visit Caulfield after someone leaves Caulfield Hospital?
We do not work on the hospital wards. Once the person is home at a Caulfield address we can cover, we can provide in-home support. Call us, or ask the discharge planner to send a referral.
Can a physiotherapist come to a home in Caulfield?
Yes. Physiotherapy at home is one of our allied health visits. You do not need a referral to book privately or through aged care or NDIS funding. A GP referral is needed if you want to claim Medicare rebates under a Chronic Condition Management Plan.
Is Konex Care a registered NDIS provider?
Yes. Konex Care is a registered NDIS provider (Registration No. 4050161035), so we can support agency-managed, plan-managed and self-managed participants.
